What happens when my patient joins?
An RN will call your patient and talk about their condition and overall health. We will help coordinate any additional needs and services that your patient may require. Your office will be sent a notification letter welcoming your patient to our disease management program.
After the first call, we will periodically check in to see how your patient is doing and answer any questions. We will give them educational information on their health condition and support to help them live a healthy lifestyle. Below are some possible topics:
- Healthy weight
- Exercising
- Planning meals
- Quitting smoking
