Requests for authorizations of member benefits are coordinated by our Utilization Management department and Long Term Support Service Coordinators. For LTSS requests, use the outpatient authorization form and fax requests to 877-794-7954.
For all other outpatient services (excluding LTSS), use the outpatient authorization form and fax requests to 800-827-7192.
