Get Rewarded for Your Health
It’s easy to earn rewards! Complete an eligible screening, exam, or vaccination and earn a gift card that you can use like a credit card.
To receive your reloadable gift card:
- Call your doctor and schedule an appointment.
- You must go to your appointment.
- After your appointment, your doctor must submit a claim.
- Processing your reward takes 8–10 weeks after we receive the claim from your doctor.
Make sure to keep your rewards card in a safe place! This card is reloaded each time you earn a reward.
As you earn more rewards, the reward money will be added to your account and you can keep using the same card.
Rewards cannot be used to buy tobacco, alcohol, or firearms. Rewards cannot be converted to cash. You can only receive one reward per applicable screening, exam, or vaccine.

Frequently Asked Questions
How does the rewards program work?
Virginia Premier Advantage Elite (HMO D-SNP) members can earn healthy rewards on a reloadable gift card by completing one or more preventive screenings, exams, or vaccinations.
To get started, schedule an appointment with your in-network doctor for your Annual Wellness Visit and Annual Physical Exam to discuss screenings, tests, and vaccines you may need.
When you complete the screening, exam, or vaccine, a claim is submitted to us by your doctor. Once Virginia Premier receives the claim, a gift card will be mailed to you within 8–10 weeks. The gift card is mailed to the address we have on record.
What are the eligibility criteria?
All members are eligible for at least four rewards: Annual Wellness Exam, Annual Physical Exam, Annual Flu Vaccine, and completion of the Health Risk Assessment. Some members may qualify for additional rewards, such as Breast Cancer Screening and Colorectal Cancer Screening. Please contact your doctor to see if you need these screenings. Members with diabetes can also earn a reward for completing a Diabetic Eye Exam, a Diabetic Kidney Function Test, or Diabetic HbA1c Test. Members eligible for Medication Therapy Management Program may also qualify for a reward.
Why should I participate in the 2022 Healthy Rewards Program?
You can get up to $230 for taking actions to stay healthy. Preventive care and staying on top of your health are important. The rewards activities selected can help you manage your overall wellness and help you develop a roadmap to achieve your health goals.
How can I follow up on my rewards eligibility or gift card status?
For questions about the rewards program or to follow up on gift card status, please call Virginia Premier Member Services at 1-877-739-1370 (TTY: 711). We are open October 1 to March 31, 7 days a week, 8:00 am to 8:00 pm, and from April 1 to September 30, Monday through Friday, 8:00 am to 8:00 pm. After business hours and on weekends and holidays our automated phone system will answer your call.
