Get Rewarded for your Health

It’s easy to earn healthy rewards! As a valued Virginia Premier Medicare member, you can earn rewards for completing health-related activities that promote your health and wellbeing. Your rewards can be used to purchase items using your prepaid flex card.

To receive your reloadable gift card:

  1. Call your doctor and schedule an appointment.
  2. Complete the appointment, screening, or exam.
  3. Your doctor will submit a claim showing you completed the preventive screening or exam.
  4. We process your reward! Processing your reward takes 8–10 weeks after we receive the claim from your doctor.

Your Prepaid Flex Card

Your prepaid flex card works just like a prepaid credit card. As a Virginia Premier Medicare member, upon earning your first reward, you will receive your reloadable prepaid flex card in the mail.

Be sure to keep your prepaid flex card in a safe place! As you earn more
rewards, the reward money will be added to your account and you can keep using
the same card all year.

The Healthy Rewards program and Grocery allowance1, 2 are offered as two standalone programs in the same card. You can pay for eligible items with your funds through one centralized card.

1 The reward given for completing the Annual Wellness Visit, Health Risk Assessment, and Medication Therapy Management program has additional restrictions and can only be used for grocery items.

2 Processing your reward takes 8–10 weeks after we receive the claim from the doctor. Rewards cannot be used to buy tobacco or alcohol. Rewards cannot be converted to cash. You can only receive one reward per applicable service per plan year. Services must be completed in 2023 using in-network providers.

Frequently Asked Questions

How does the rewards program work?

Virginia Premier Advantage Elite (HMO D-SNP) members can earn Healthy Rewards on a prepaid flex card by completing one or more preventive screenings, or exams.

To get started, schedule an appointment with your in-network doctor for your Annual Wellness Visit and Annual Physical Exam to discuss screenings, tests, and vaccines you may need.

When you complete the screening, exam, a claim is submitted to us by your doctor. Once Virginia Premier receives the claim, your reward funds will be added to your prepaid flex card within 8-10 weeks.

What are the eligibility criteria?

All members are eligible for the Annual Wellness Exam, Annual Physical Exam, and completion of the Health Risk Assessment. Some members may qualify for additional rewards, such as Breast Cancer Screening, Colorectal Cancer Screening, and Bone Density Scan. Please contact your doctor to see if you need these screenings. Members with diabetes can also earn a reward for completing a Diabetic Eye Exam, a Diabetic Kidney Function Test, or Diabetic HbA1c Test. Members eligible for Medication Therapy Management Program may also qualify for a reward.

If you are enrolled in Virginia Premier and want to complete the Health Risk Assessment, you can complete your HRA by phone at 1-877-719-7358 (TTY:711), October 1 – March 31, 7 days a week, 8 a.m. – 8 p.m. and April 1 – September 30, Monday – Friday, 8 a.m. – 8 p.m.

If you take eight or more medications for more than one chronic health condition and want to see if you are eligible for a Medication Therapy Management program, contact the ESI Pharmacy Help Desk at 1-800-922-1557 (TTY: 711) Monday – Friday, 8 a.m. – 5 p.m.

What is the prepaid flex card?

The Healthy Rewards Program and Grocery Allowance are offered as two standalone programs on the same card. You can pay for eligible items with your funds through one centralized card. Be sure to keep your prepaid flex card in a safe place. As you receive more funds—through your monthly Grocery Allowance and any Healthy Rewards you earn—the money will be added to your account, and you can keep using the same card all year.

The benefit you get from participating in the Healthy Rewards program is more than just earning reward funds. Preventive care and staying on top of your health are important. The rewards activities selected can help you manage your overall wellness and help you develop a roadmap to achieve your health goals.

How can I ask about my eligibility or to check on the status of my earned rewards?

For questions about the Healthy Rewards Program or to follow up on gift card status, please call Virginia Premier Member Services at 1-877-739-1370 (TTY: 711). We are open October 1 to March 31, 7 days a week, 8:00 am to 8:00 pm, and from April 1 to September 30, Monday through Friday, 8:00 am to 8:00 pm. After business hours and on weekends and holidays our automated phone system will answer your call.
