How to Get a New Member ID Card

Your Virginia Premier member ID card is an important part of your health care. It’s what lets doctors, hospitals and other providers know Virginia Premier is going to cover the services you get.

But, like any pocket-size card, sometimes you misplace it or it goes missing. There’s no need to worry, though. You can always get a new card from Virginia Premier through Member Services or your Member Portal. Simply find your plan below and use the information specific to your plan:

How to Replace Your Virginia Premier Member ID Card*
  • Elite Individual – Medallion 4.0
  • Elite Family (FAMIS) – Medallion 4.0
  • Call 1-800-727-7536 (TTY: 711), and have your Medicaid ID number ready, or
  • Log in to the Member Portal (only temporary cards can be printed from the Member Portal)
  • Elite Plus (CCC Plus)
  • Call 1-877-719-7358 (TTY: 711), and have your Medicaid ID number ready, or
  • Log in to the Member Portal (only temporary cards can be printed from the Member Portal)
  • Virginia Premier / Kaiser Permanente Elite Individual – Medallion 4.0
  • Call Kaiser Permanente Member Services at 1-855-249-5025
  • Log in to the Member Portal
Medicare Advantage
  • Virginia Premier Advantage Gold (HMO)

In order to register for an account, if you would like access to all group NPIs/APIs under a single Tax ID, please enter your Tax ID only. If you would like access to a specific group NPI/API instead, please enter it in the field provided. 

For expedited registration, please provide a Claim number and the Total Billed Charges on the claim when creating your account.

You will receive an email within 3-5 business days with your username and temporary password.
