As a member of Virginia Premier who is pregnant, we provide you with a medical outreach representative to help you out. But you may also be able to benefit from our case management team. What’s the difference between the two?
Well, your medical outreach representative will help you get the most out of your Virginia Premier benefits. For example, they’ll introduce you to our prenatal Healthy Heartbeats program. Healthy Heartbeats provides education – and rewards – related to ways to keep you and your baby healthy during your pregnancy.
Case management
A case manager can provide even more support for pregnant members, particularly those who have a high-risk pregnancy.
Our case managers include registered nurses and licensed social workers. Case managers meet regularly with members to plan their care both before and after they give birth.
For example, case managers help members keep track of all their medications, and help ensure that their baby is signed up for Medicaid if need be. And they can refer members to other helpful programs, including health education, chronic care management and more.
